WACTE Sam Hitchcock Memorial Scholarship Application

Instructions:

The scholarship application contains two parts.  First is general demographic information and school accomplishments.  Next are three essays which the committee will use to determine the scholarship recipient.  Each page should include a header that contains the applicant's last name and last four digits of his/her social security number (i.e. Smith 1234).  Use Times New Roman, 12pt. font, double-spaced for all pages.  Please use a new page for each section.

Please note that the demographic information gathered is strictly for statistical reasons.  The information will not be sold to outside companies nor used as a determinant in the final decision process.

Section 1:  General Information:

On the first page, please include the following:

    -Name (First MI Last), Age, Gender
    -Local Address
    -Local Phone
    -Permanent Address
    -Permanent Phone
    -Parent's Name(s)
    -Parent's Address
    -Parent's Phone

Section 2:  School Accomplishments:

    -High School Graduated From and Date of Graduation
    -Cumulative GPA
    -Hours Earned
    -Extracurricular Activities (include any offices held)
    -Work experience (if applicable)

Essay 1 (750 word maximum):

What are your beliefs and philosophies in regard to career and technical education?  What do you feel about its current state?  What kind of changes would you like to see locally?  Regionally?  Nationally?  What traits are important for a teacher to be successful in the classroom?

Essay 2 (500 word maximum):

Where do you see yourself in the next five to ten years?  Where would you like to teach?  What area?

Essay 3 (500 word maximum):

How has career and technical education affected you personally?

Essay 4 (optional; 250 word maximum)

Please include any other information you feel would be helpful for the review committee in making its decision, such as financial need.
